About the Role

We are seeking a skilled Certified Nurse Practitioner to join our team at Nurse Next Door. In this rewarding role, you will provide high-quality care to our clients in their own homes, ensuring they receive the best possible support and treatment.

Job Summary:

The successful candidate will be responsible for delivering patient-centered care, working collaboratively with our multidisciplinary team to develop and implement individualized plans of care. You will also conduct thorough assessments, administer medications, and provide education and support to clients and their families.

Key Responsibilities:

  • Deliver high-quality patient care, adhering to evidence-based practices and regulatory standards.
  • Conduct comprehensive assessments to identify client needs and develop personalized care plans.
  • Administer medications, treatments, and therapies as prescribed by healthcare professionals.
  • Provide education and support to clients and their families, promoting independence and empowerment.
  • Collaborate with interdisciplinary teams to coordinate care and services.
  • Document client information accurately and confidentially.

Requirements:

  • Degree in Nursing (LPN or RN) from an accredited institution.
  • Certification as a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
  • Minimum 1 year of experience in healthcare, preferably in home care or gerontology.
  • Strong communication and interpersonal skills, with ability to work effectively in a team environment.
  • Compassion, empathy, and a commitment to delivering exceptional patient care.

Estimated Salary: $60,000 - $80,000 per annum
