POSITION SUMMARY:
In collaboration with the designated Clinical Team Manager, the Licensed Practical Nurse (LPN) is accountable for delivering proficient nursing care to patients and their families. This role involves coordinating care with other members of the home health team and overseeing Home Health Aides. A significant portion of patient interaction is dedicated to evaluations and informational consultations.
KEY RESPONSIBILITIES:
- 1. Monitors and communicates any signs or symptoms indicating changes in the patient's condition or family dynamics to the facility supervisor, ensuring all observations are documented in the clinical record.
- 2. Executes assigned nursing tasks, including administering treatments and medications in accordance with the State Nurse Practice Act and the established Plan of Care.
- 3. Prepares necessary equipment and materials for treatments while adhering to aseptic techniques as required.
- 4. Aids in the rehabilitation process of patients following the individualized care plan.
- 5. Upholds the confidentiality of medical records at all times.
- 6. Diligently documents all patient-related events and completes all necessary paperwork as mandated by state and/or federal regulations.
- 7. Alerts the team leader to any changes in the patient's condition and ensures proper documentation.
- 8. Engages in in-service education programs as mandated by state law.
- 9. Completes all relevant documentation related to patient care, including updates.
- 10. Consistently demonstrates proficiency in technical nursing skills in line with the legal scope of practice.
- 11. Under the supervision of a Registered Nurse, may undertake supervisory responsibilities in home care, specifically within programs managed by First Choice Medical Staffing.
- 12. Performs additional related duties and responsibilities as necessary.
