This role is pivotal in providing comprehensive support to adults managing diabetes, including those newly diagnosed and those already engaged in our educational initiatives.
Key Responsibilities
- Conduct assessments to identify the educational needs of patients and their families.
- Deliver thorough diabetes education and nutritional guidance in both individual and group settings.
- Facilitate patient education on advanced insulin management and insulin pump usage.
- Support pregnant patients with diabetes, offering tailored education for various diabetes types.
- Engage in all facets of clinical care, including direct patient interactions and case management.
- Design and evaluate personalized care plans for diabetes patients, monitoring their progress closely.
- Provide follow-up communication with patients and collaborate with healthcare providers.
- Assist in the development and evaluation of diabetes self-management education programs.
- Manage patient communications effectively, including triaging inquiries from various stakeholders.
- Maintain accurate documentation and adhere to clinical standards of care.
Qualifications
- Bachelor's degree from an accredited institution.
- Completion of an accredited dietetics program and supervised practice.
- Active registration as a Registered Dietitian Nutritionist.
- Certification as a Diabetes Care and Education Specialist or commitment to obtain within two years.
- Experience in diabetes education is essential.
Preferred Skills
- Familiarity with electronic health record systems.
- Experience with insulin pump and continuous glucose monitoring systems.
