Job Summary:

The Physical Therapist will be responsible for planning, administering, and evaluating physical therapy treatment programs for patients in their homes. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans that address the unique needs and goals of each patient.

Responsibilities:
  • Plan and administer physical therapy treatment programs for patients in their homes
  • Conduct thorough assessments of patients' physical strengths and limitations
  • Develop and implement individualized treatment plans that address patients' unique needs and goals
  • Collaborate with our interdisciplinary team to ensure seamless care coordination
  • Document patient progress and treatment plans accurately and thoroughly
  • Communicate effectively with patients, families, and healthcare team members
Requirements:
  • Doctor of Physical Therapy (DPT) degree from an institution accredited by the American Physical Therapy Association (APTA)
  • Current and unrestricted license to practice Physical Therapy in the State of California
  • Current CPR/BLS certification
  • Compliant with state healthcare regulatory requirements
  • Ability to care for patients with different personalities and needs
  • Ability to assess patient condition and work with treatment plans
  • Minimum of 1 year of recent Physical Therapy experience in an institutional setting
  • Minimum of 1 year of recent Physical Therapy experience with a home health agency
