Job Summary:

Trilogy Home Healthcare Clearwater Office is seeking a skilled Registered Nurse to provide high-quality care to patients in their homes. As a Registered Nurse, you will be responsible for managing the medical needs of our patients, working with other healthcare professionals to maximize patient outcomes, and advocating for patients' needs.

Key Responsibilities:

  • Provide comprehensive nursing care to patients in their homes, including assessing patients' medical needs, developing care plans, and implementing treatment plans.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care for patients.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient care and progress in a timely and accurate manner.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.

Requirements:

  • Graduate of an accredited school of nursing and currently licensed in the state(s) in which practicing.
  • 1 year of nursing experience preferred.
  • Ability to exercise initiative and independent judgment.
  • Strong communication and interpersonal skills.
