Job Summary

CarePartners Health Services is seeking a dedicated Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in providing high-quality patient care and improving patient outcomes.

Responsibilities
  • Manage an assigned caseload of patients, providing therapy and clinical case management to improve patient outcomes and manage resources effectively.
  • Conduct comprehensive assessments, develop and modify plans of care in conjunction with patients, caregivers, and physicians to achieve patient goals and clinical outcomes.
  • Provide treatment visits and discharge planning to facilitate patient engagement and achieve positive patient experiences.
  • Communicate handoff information and coordinate with Physical Therapist Assistants, other therapists, and members of the care team to support patient care throughout the home health continuum.
Requirements
  • Current North Carolina license to practice Physical Therapy.
  • Driver's license required.
  • Basic Cardiac Life Support certification within 7 days of employment start date.
  • One to two years of related experience.
  • Preferred experience in clinical affiliations with a wide variety of acute care procedures, especially orthopedics, neurology, cardiopulmonary, and care of burns/open wounds.
