Della Infotech, Inc. is seeking a qualified Community Health Nurse to provide home infusion therapy services in Pittsfield, MA. This is a great opportunity for a skilled nurse to work independently and make a meaningful difference in the lives of our patients.

**Job Summary**
The successful candidate will be responsible for providing high-quality nursing care to patients in their homes, focusing on infusions and other related treatments. As a Community Health Nurse, you will work closely with our interdisciplinary team to ensure seamless care coordination and exceptional patient outcomes.

**Key Responsibilities**
  • Provide direct patient care, including administering medications, monitoring vital signs, and documenting progress.
  • Develop and implement individualized care plans, taking into account each patient's unique needs and circumstances.
  • Collaborate with other healthcare professionals to ensure comprehensive care and address any concerns or issues that arise.
  • Maintain accurate and up-to-date records, adhering to all relevant regulations and guidelines.

**Requirements**
  • Bachelor's degree in Nursing (BSN) required; master's degree preferred.
  • Minimum one year of experience in acute nursing care, preferably in a hospital setting.
  • Proven expertise in home health and infusion therapy, with a strong understanding of relevant principles and practices.
  • Certified Home Health Required Including Proficiency with OASIS; IV Certification Required/1 Year IV Experience preferred.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and colleagues.
  • Strong organizational and time management skills, with the capacity to prioritize tasks and manage competing demands.
  • Current Driver License (and insurance) Required.
