Job Summary

We are seeking a skilled Case Manager to join our team at RCIL in Utica, NY. As a Case Manager, you will be responsible for assessing the needs of individuals with disabilities and developing personalized plans to support their growth and development.

Key Responsibilities
  • Conduct thorough risk assessments to identify potential crisis situations and develop strategies to prevent them.
  • Develop and implement Support Service Plans to address identified needs and promote independence.
  • Maintain regular contact with clients and their families to monitor progress and provide guidance and resources as needed.
Requirements
  • A minimum of an Associate's Degree or equivalent experience is required.
  • Strong analytical and problem-solving skills are essential for success in this role.
  • Prior experience working with individuals with disabilities is highly desirable.
Compensation and Benefits

We offer a competitive salary range of $37,856 annually, plus benefits including 16 vacation days, 12 sick days, and 13 paid holidays, as well as a 401K plan with employer matching.
