Physical Therapist for Home Health

FeldCare Connects is seeking a motivated and compassionate Physical Therapist to deliver high-quality home healthcare to patients of all ages in La Jolla and surrounding areas.
Key Responsibilities- Provide one-on-one treatment to patients in their homes, observing, recording, and reporting their response to treatment and changes to their condition.
- Determine the plan of care for each patient, working independently as an independent contractor.
- Utilize our Electronic Medical Record system for easy documentation, completing records during visits when possible.
- Access patients through our innovative app Cliniconnects, accepting or declining patients in seconds.
- Current Physical Therapist license issued by the state Board of Physical Therapy, unrestricted and active.
- Completion of an accredited Physical Therapy program.
- Reliable personal transportation and a smartphone.
- Bilingual a plus.
FeldCare Connects is a referral network that connects clinicians with patients in their community, contracting with over 900 home health agencies throughout CA, AZ, TX, and FL.
