Physical Therapy Assistant

**Job Summary:**
We are seeking a skilled Physical Therapy Assistant to join our team in a sub-acute/skilled nursing facility setting. As a Physical Therapy Assistant, you will work collaboratively with other rehab therapists and nursing staff to provide comprehensive and compassionate therapy services to our residents.

**Responsibilities:**
* Provide physical therapy services to residents in a sub-acute/skilled nursing facility setting
* Work collaboratively with other rehab therapists and nursing staff to develop and implement individualized treatment plans
* Conduct thorough assessments of residents' physical abilities and develop treatment plans to improve their functional mobility and independence
* Implement and modify treatment plans as needed to ensure optimal resident outcomes
* Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination
* Maintain accurate and timely documentation of resident progress and treatment plans

**Requirements:**
* Current Physical Therapy Assistant (PTA) license or be license eligible to practice in the state employed
* Strong computer skills with Microsoft Outlook, Word, Excel, and experience with electronic documentation
* Good attention to detail and timeliness with documentation
* Ability to work well in a team setting and be flexible with hours
