Key Responsibilities
  • Conduct thorough assessments of patients in their homes, including medical history, diagnosis, and treatment plans
  • Develop and implement personalized care plans to address identified health risks and promote healthy outcomes
  • Collaborate with primary care physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and self-management
  • Document all assessments, care plans, and patient interactions in accordance with Matrix Medical Network's policies and procedures
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in state of practice, with prescription authority
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent verbal and written communication skills
  • Ability to travel to patient homes and other locations as needed
