Social Worker, MSW

We are seeking a skilled Social Worker, MSW to join our team as a Patient Care Specialist. In this role, you will work collaboratively with the care team to facilitate and coordinate services for patients while admitted and post-discharge.
As a Social Worker, MSW, you will assess patients' and families' needs, provide psychosocial support, and link them to relevant resources. Your expertise will help ensure seamless transitions and optimal patient outcomes.
Responsibilities include:
- Supporting campus and system goals for capacity management through length of stay goals and Case Management initiatives
- Completing comprehensive assessments of patient situations using Case Management documentation standards
- Recognizing problems, gathering data, and making decisions to formulate appropriate treatment plans
Requirements:
- Master's Degree in Social Work from an accredited program
- LISW license in the State of Ohio required
- Two years minimum of recent discharge planning experience in an acute care setting and knowledge of basic medical terminology and basic computer skills
