Nurse Care Manager

We are seeking a highly skilled and compassionate Nurse Care Manager to join our team at Evergreen Nephrology. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to individuals with kidney disease.
Key Responsibilities- Collaborate with nephrologists, advanced practice providers, and other healthcare professionals to develop and implement comprehensive care plans for patients with kidney disease.
- Conduct thorough assessments to identify patients' needs, values, and goals, and develop personalized care plans to address their unique needs.
- Coordinate patient care delivery with Evergreen Nephrology's interdisciplinary team, network providers, and community-based services.
- Monitor patient outcomes and make recommendations for cost-effective measures to improve quality of care and reduce healthcare utilization.
- Act as a clinical liaison with outside agencies, such as County CCS, non-plan facilities, and third-party administrators, to ensure seamless care transitions and coordination.
- Prepare reports, communicate program changes to staff, and develop protocols in accordance with compliance requirements.
- Current RN license required; BSN preferred.
- 1+ years of utilization management experience preferred.
- 3+ years of clinical practice in a hospital, clinic, home care, or nursing home setting highly preferred.
- Case management experience and CCM preferred but not required.
