Millennium Home Care, an affiliate of Millennium Healthcare, LLC, is seeking a PRN Physical Therapist Assistant to join our dynamic team.
About the RoleAs a Physical Therapist Assistant, you will have the unique opportunity to provide exceptional care to patients in the comfort of their own homes.

Responsibilities- Provide high-quality, evidence-based physical therapy services to patients in their homes, under the guidance and supervision of a licensed Physical Therapist.
- Assist in the development and implementation of patientspecific treatment plans, focusing on promoting mobility, independence, and overall physical wellbeing.
- Conduct comprehensive assessments to identify patients' needs, goals, and potential barriers to recovery.
- Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to ensure integrated and holistic patient care.
- Educate patients and their families on therapeutic exercises, adaptive techniques, and strategies to promote independence and enhance overall functioning.
- Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
- Continuously stay updated on the latest advancements in physical therapy techniques, ensuring the delivery of cutting-edge and evidence-based care.
- Adhere to all regulatory and compliance standards, ensuring the highest level of patient safety and confidentiality.
- Current state license as a Physical Therapist Assistant in Florida.
- A Minimum of 1 year of experience in home health, or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
