As a Clinical Pharmacist in our Inpatient Cardiology Unit, you will be part of a collaborative team that shapes the future of healthcare.

About the Role

We are seeking a highly skilled and motivated Clinical Pharmacist to join our team in the Inpatient Cardiology Unit at the Hospital of the University of Pennsylvania (HUP). As a Clinical Pharmacist, you will be responsible for providing pharmaceutical care to patients, including medication therapy management, patient education, and collaboration with healthcare teams.

Responsibilities
  • Prepares and dispenses medications using appropriate techniques to ensure outcomes of drug therapies and following the health system's policies and procedures
  • Provides concise, applicable, and timely responses to drug information requests from healthcare providers and patients
  • Ensures continuity of pharmaceutical care to and from the acute and ambulatory patient-care settings
  • Participates in the established process for assessing, managing, and reporting Adverse Drug Events (ADEs)
  • Participates in the management of medical emergencies
  • Assumes responsibility for the adherence to medication-use policies and guidelines
  • Manages time effectively to fulfill practice responsibilities
  • Complies with accreditation, legal, regulatory, and safety requirements for a specific practice setting
Requirements
  • Current pharmacist license in the state of Pennsylvania or eligible for licensure (required)
  • Bachelor of Science in Pharmacy or Doctor of Pharmacy from an accredited College of Pharmacy (required)
  • PGY-1 Pharmacy Practice Residency or 2 years of practice (preferred)
  • PGY-2 Pharmacy Practice Residency (preferred)
