Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Ascension Healthcare. As a LPN, you will provide high-quality patient care and support to our long-term care patients under the direction of a Registered Nurse (RN) or physician.

Key Responsibilities
  • Contribute to the assessment of patients by collecting data for analysis by a Registered Nurse (RN).
  • Administer medications and observe patients for adverse reactions to medications or treatments.
  • Perform routine laboratory tests and simple therapeutic services.
  • Provide education to patients and families about disease treatment plans, including self-care post-discharge, holistic health needs, available resources, and follow-up care.
  • Ensure resident safety by identifying safety hazards and emergency situations and initiating corrective action as needed.
Requirements
  • BLS Provider certification obtained within 1 month of hire date or job transfer date required.
  • Licensed Practical/Vocational Nurse licensure obtained prior to hire date or job transfer date required.
  • High School diploma equivalency or 1 year of applicable cumulative job-specific experience required.
