Job Summary:

Liberty Cares is seeking a dedicated and compassionate Hospice Registered Nurse to join our team. As a Hospice Nurse, you will provide high-quality patient care and support to individuals and their families facing terminal illness.

Key Responsibilities:
Apply a working knowledge of nursing theories and concepts to deliver patient-centered care.
Coordinate physical care of the patient with family, patient, physician, and Hospice team.
Communicate effectively with patients, families, and healthcare professionals.
Participate in Interdisciplinary Team (IDT) meetings and quality reviews to ensure seamless care delivery.

Requirements:
Current RN licensure in the state of practice.
Successful completion of a nursing education from an approved school of nursing with two years of clinical nursing experience and at least one year of hospice nursing experience preferred.
Knowledge of home health and hospice care programs and the nurse's role as a member of the patient care team.
12 hours of continuing education annually and CPR certification.
Ability to accept travel assignments and weekend/evening and holiday call duties.
