Compensation Rates:

Role Overview:
The Practical Nursing Specialist is accountable for overseeing the implementation of resident care plans and supervising daily care activities during assigned shifts. This role encompasses both care management and supervisory duties, ensuring compliance with all relevant regulations and organizational policies.
Key Responsibilities:
- Direct and manage daily resident care activities.
- Ensure adherence to nursing regulations and facility policies.
- Collaborate with interdisciplinary teams to coordinate comprehensive resident care.
- Document resident needs and communicate effectively with the next shift's RN Supervisor.
- Supervise Certified Nursing Assistants in delivering care and maintaining standards.
- Complete necessary medical documentation and participate in care plan meetings.
- Facilitate the admission, transfer, and discharge processes for residents.
- Monitor and provide care for residents with significant health concerns.
Qualifications:
- Completion of a nursing program from an accredited institution.
- Minimum of one year of experience in long-term care or acute care settings.
- Current NYS LPN license and CPR certification required.
- Compliance with vaccination requirements for the health and safety of residents.
