Responsibilities:
  • Deliver Patient Care: Provide basic patient care, including personal care, assistance with meals, mobility assistance, light housekeeping, and companion services.
  • Assess Patient Needs: Evaluate the safety of the delivery of care in the home environment and recognize changes in the patient's condition, communicating with the RN supervisor as needed.
  • Implement Care Plans: Provide care in compliance with the assigned care plan and RN delegation, ensuring that all applicable laws, regulations, and policies are met.
  • Maintain Patient Records: Accurately document patient information, including vital signs, medication administration, and any changes in the patient's condition.
  • Collaborate with Healthcare Team: Work closely with the RN supervisor, other healthcare professionals, and family members to ensure seamless care and support.
Requirements:
  • Education: High school diploma or equivalent required.
  • Experience: At least 18 years of age and 1 year of experience in a healthcare setting preferred.
  • Certifications: Current CPR certification and HHA licensure in the State of New York required.
