Job Summary:We are looking for a compassionate and experienced Physical Therapist to join our team. As a Physical Therapist with VNA Health Group, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals.
Responsibilities:- Assess and evaluate patients for physical therapy needs and develop individualized plans of care.
- Provide skilled physical therapy services to patients in their homes, including exercises, stretches, and other interventions.
- Monitor patient progress and adjust treatment plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
- Document patient information and treatment plans accurately and thoroughly.
- Physical Therapy licensure in NJ required.
- 2-3 years of prior clinical experience and community-based experience preferred.
- Strong computer skills and ability to use electronic medical records.
- Excellent communication and interpersonal skills.
- Valid NJ driver's license.
