Physical Therapist

Care IV Home Health is seeking a skilled Physical Therapist to join our team. As a Physical Therapist with our agency, you will have the opportunity to work in a professional and supportive environment that fosters flexibility, work-life balance, and meaningful patient relationships.
Responsibilities- Visit patients in their homes or assisted living facilities to provide physical therapy care.
- Develop and execute personalized physical therapy care plans for each patient.
- Record patient progress, including admissions, discharges, and regular updates to the Director.
- Complete other assigned tasks as needed.
- Active Arkansas Physical Therapy license or registration in the state of Arkansas.
- A minimum of three professional references.
- Home Health experience is a plus, but not required.
