Position Overview:

The Nurse Practitioner is tasked with conducting in-person evaluations of patients receiving hospice care across various environments. This role involves collaboration with the Physician and team members to determine patient eligibility for hospice services and to provide necessary interventions for effective pain and symptom management, showcasing a high level of expertise in patient care. The position operates under the direct oversight of the Hospice Physician or the Hospice Medical Director.

Qualifications:

  • Master's Degree in Nursing
  • Licensed and certified as a Nurse Practitioner in the practicing state
  • Possession of a DEA License in the practicing state
  • In good standing and eligible for participation in Medicare & Medicaid programs
  • Preferred minimum of 5 years of nursing experience; clinical experience in oncology, hospice, geriatrics, or intensive care is advantageous

Key Responsibilities:

  • Responsible for managing a designated caseload of patients according to an established care plan.
  • Complies with privacy and security regulations as mandated by federal, state, and local authorities, including HIPAA.
  • Delivers care, treatment, and services that align with professional standards, guidelines, laws, and regulations.
  • Creates and executes a detailed, patient-centered care plan for each individual, updating it based on physician orders while ensuring patient engagement and comprehension.
  • Refers patients to additional services as necessary.
  • Maintains accurate clinical documentation in accordance with agency policies and accepted nursing standards.
  • Performs additional duties as assigned.
