Key Attributes Desired:

Required Licenses/Certifications:

Registered Dietitian/Nutritionist (RD/RDN)

Main Responsibilities:
  • Educate elderly patients on the use of our mobile application and medical devices, assisting with any technical difficulties.
  • Collaboratively create personalized care plans, behavior goals, and educational objectives for each patient while ensuring HIPAA compliance.
  • Implement established Standards of Practice and Professional Performance in diabetes care to guide educational interventions.
  • Work in conjunction with primary care physicians and other healthcare professionals.
  • Utilize iHealth Unified Care software and patient vitals to monitor outcomes and set educational goals.
  • Provide ongoing support to patients through our online platform during scheduled shifts.
  • Collaborate with a cross-functional team to enhance digital health solutions for chronic disease management.
  • Potentially mentor peers within the clinical service team and contribute to the development of clinical protocols.
  • Create educational materials such as blog posts, videos, and handouts that align with AADE and AHA standards.
  • Perform additional duties and projects as assigned.
