About the Role

Key Responsibilities
  • Education and Training: Educate prospective patients on the benefits of home dialysis and provide comprehensive training on their chosen modality, ensuring they have the skills and confidence to perform their treatments safely and effectively.
  • Treatment Planning: Collaborate with our Nephrologists to develop personalized treatment plans that meet the unique needs of each patient, taking into account their medical history, lifestyle, and treatment goals.
  • Patient Support: Provide ongoing support and guidance to patients and their families, addressing any concerns or questions they may have and helping them to overcome any challenges they may face.
  • Team Collaboration: Work closely with our multidisciplinary team to ensure seamless communication and coordination of patient care, ensuring that patients receive the highest level of service and support.
  • Continuous Improvement: Stay up-to-date with the latest technologies and evidence-based practices, sharing your knowledge and expertise with the team to drive continuous improvement in patient care.
Requirements
  • Current RN License: Current Registered Nurse (RN) license in the state of practice.
  • Experience: 12 months of experience in nursing, with an additional 3 months of experience with specific modality.
  • Education: Associates Degree in Nursing (ADN) from an accredited school, Bachelor of Science in Nursing (BSN), or three-year diploma from an accredited program.
  • Communication Skills: Excellent communication and interpersonal skills, with the ability to effectively communicate with patients, families, and healthcare professionals.
  • Teamwork: Ability to work collaboratively as part of a team, with a strong focus on patient-centered care.
