Are you ready for a rewarding and challenging role as a Hospice Nurse? We are in search of compassionate, dynamic, and organized individuals with hospice experience to join our Hospice Family serving clients in the local community.

In this position, you will be a registered nurse who understands the significance of hospice care. Your role will involve providing emotional support to help patients maintain dignity and comfort during their final days. Our Hospice Nurses will conduct essential supervisory visits to ensure quality care and participate in interdisciplinary team meetings to develop and refine patient care plans.

Key Responsibilities:

  • Conduct initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
  • Implement and adjust individualized care plans as necessary.
  • Adhere to all hospice policies and Medicare regulations.
  • Educate patients and families on disease processes, self-care techniques, and end-of-life considerations.
  • Supervise paraprofessionals providing patient services.
  • Document clinical and progress notes and engage in interdisciplinary group discussions.
  • Contribute to hospice performance improvement initiatives.
  • Maintain high levels of satisfaction among clients, families, and caregivers.
  • Communicate effectively using current organizational processes and technologies.

Qualifications:

  • Current Michigan Nursing License in good standing.
  • Minimum of one year of hospice experience.
  • Empathetic approach towards patients and their families.
  • Willingness to participate in field visits and team meetings.
  • Ability to adapt to changing patient needs on a full-time on-call basis.
