Job Summary:- Provide primary medical care to participants, including assessment, development of a plan of care, and evaluation of effectiveness.
- Function in a collegial relationship with other healthcare professionals, making independent decisions regarding medical needs and health regimens.
- Carry out medical procedures as indicated.
- Deliver high-quality medical care to elderly patients, including routine check-ups, chronic disease management, and acute care.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
- Board Certification in Family Medicine or Internal Medicine.
- MA license (or willing to obtain).
- CDS Certification & DEA Verbal Confirmation Registration (prior to start).
- Minimum 1 year working with the frail or elderly population.
- BLS (prior to start).
- Clean/Clear malpractice.
- EMR TruChart experience.
