Speech Language Pathologist

We are seeking an experienced Speech Language Pathologist to join our team of rehabilitation professionals in our Home and Community Program. As a member of our team, you will have the opportunity to work with patients in their natural environment, providing functional rehabilitation and promoting independence.
About the RoleAs a Speech Language Pathologist in our Home and Community Program, you will be responsible for:
- Performing evaluations and developing treatment plans for patients with speech, language, cognitive, and swallowing disorders
- Regularly reassessing the effectiveness of treatment plans and attending staffing meetings and family conferences
- Performing discharge evaluations and making recommendations for home and community safety
- Communicating patient needs and progress to the treatment team, physician, and family members
To be successful in this role, you will need:
- A Master's Degree from an accredited Speech Language Pathology program
- A valid Speech Language Pathology license in the state(s) of practice
- Current CPR Certification in accordance with state regulations
- A minimum of one year's work experience as a Speech Language Pathologist
