The Nurse Practitioner is responsible for providing primary healthcare services to patients of Evergreen Health's Primary & Specialty Care practice. This role involves working as part of a multidisciplinary team to provide patient-centered care.
Key responsibilities include:
- Assessing the physical and psychosocial status of patients through interview, health history, physical examinations, and diagnostic tests.
- Evaluating test findings, making initial medical diagnoses, and developing therapeutic plans consistent with ongoing patient needs.
- Recording findings of physical and developmental assessments in a timely and accurate manner.
- Identifying abnormalities requiring further study and proceeding with follow-up care.
- Initiating first-line medical care plans within defined protocols and discontinuing therapeutic regimens as needed.
- Developing primary care health regimens utilizing a multidisciplinary team approach.
- Making referrals to other healthcare providers and consulting with or referring patients to physicians as per treatment protocols.
- Providing emergency care services in life-threatening situations as per protocol.
