Responsibilities

As an Occupational Therapist with HarmonyCares, you will be responsible for evaluating the level of function of our patients and developing a plan of treatment to help them achieve their goals. Your duties will include:
  • Evaluating patients' level of function and developing treatment plans
  • Guiding and instructing patients in therapeutic activities to improve their independence and physical and mental functioning
  • Advising and consulting with patients and their families on the use of prosthetic, orthotic, assistive devices, and adaptive equipment
  • Participating in Performance Improvement Activities and discharge planning
  • Ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and providing Age-Friendly Care

Qualifications

To be successful in this role, you will need to have:
  • A graduate degree in occupational therapy from an accredited program
  • Current licensure as an Occupational Therapist in the state of practice with at least 2 years of experience
  • Active CPR certification
  • A valid driver's license, reliable vehicle, and good driving record
