Job Summary:

We are seeking a skilled Nurse Care Specialist to join our team at IntelyCare. As a Nurse Care Specialist, you will be responsible for providing high-quality patient care in a variety of settings, including assisted living facilities and skilled nursing facilities.

Responsibilities:

  • Administering nursing care to patients in accordance with their individualized care plans
  • Working collaboratively with residents, medical staff, and other healthcare professionals to devise and implement effective care plans
  • Ensuring that patients receive the right personalized treatments and medications
  • Explaining treatment options and care plans to patients and their families
  • Consulting with other healthcare professionals, including doctors, to ensure comprehensive patient care
  • Setting up and monitoring medical equipment
  • Overseeing compliance with facility policies and nursing standards
  • Documenting and verifying the accuracy of interventions and medical histories
  • Performing fluid sample draws for lab work
  • Leading training and supervision of nursing staff
  • Assisting with incident investigations and verifying the accuracy of reporting

Requirements:

  • Current Rhode Island state license to practice as a Registered Nurse (RN)
  • Completion of an approved nursing program
  • Experience in post-acute care or in-patient hospital settings (at least 4 months)
  • Excellent organizational skills
