Overview:

We are seeking a skilled Speech-Language Pathologist to join our team at Bridge Home Health and Hospice. As a Speech-Language Pathologist, you will be responsible for implementing standards of care for speech-language pathology services and adhering to all conditions in the Service Agreement.

Responsibilities:

  1. Conduct evaluations, assess home environments, and identify equipment needs relative to speech-language function.
  2. Identify functional speech defects and establish a plan of care to improve patients' function.
  3. Administer speech therapy programs utilizing specialized therapeutic techniques and equipment.
  4. Recommend communication devices and aids as indicated.
  5. Confer with referring physicians and other organization personnel to ensure coordinated and comprehensive care.
  6. Participate in case conferences, in-services, and other meetings required to ensure coordinated and comprehensive care.
  7. Prepare and submit notes per organization policy.
  8. Maintain clinical competency in speech-language pathology practice and theory.
  9. Identify patient and family/caregiver needs for other services and refer as appropriate.
  10. Instruct patients, family/caregivers, and other organization healthcare personnel in the patients' treatment regime as indicated.
  11. May supervise home health aides in procedures directly related to the speech-language therapy plan of care.
  12. Perform other duties as required to facilitate the delivery of speech-language pathology services.

Qualifications:

  • Possess a master's or doctoral degree in speech-language pathology (SLP) and meet either of the following requirements:
    • Be licensed as a speech-language pathologist by the State in which the individual furnishes such service.
    • If the State does not license speech-language pathologists:
    • Must successfully complete 350 clock hours of supervised clinical practicum (or be in the process of accumulating such supervised clinical experience).
    • Performed not less than 9 months of supervised full-time SLP services after obtaining a master's or doctoral degree in SLP or a related field.
    • Successfully completed an approved national examination in SLP.
  • A minimum of one (1) year's clinical experience in Speech-Language.
  • Demonstrate good verbal and written communication, and organization.
  • Possess and maintain current CPR certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
