About the Role

We are seeking a highly skilled and dedicated Resident Assessment Coordinator MDS to join our team at Christian Horizons. As a key member of our nursing service team, you will play a vital role in ensuring the highest quality of care for our residents.


Your primary responsibility will be to assess resident care needs and develop individualized care plans that meet their unique requirements. You will work closely with our interdisciplinary team to coordinate care conferences, conduct resident assessments, and review medical records to ensure accurate and timely submissions.


As a Resident Assessment Coordinator MDS, you will be responsible for:

  • Assessing resident care needs and developing individualized care plans
  • Scheduling and conducting resident care conferences
  • Conducting resident assessments and reviewing medical records
  • Ensuring timely and accurate submissions of MDS records
  • Working collaboratively with our interdisciplinary team to provide high-quality care
  • Ensuring compliance with federal, state, local, and corporate regulations

Requirements

To be successful in this role, you will need:

  • A graduate degree in nursing from an accredited school
  • A current state license as an LPN or RN
  • A clear understanding of MDS principles and practices
  • 1 year of experience in MDS, preferably in a long-term care setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong organizational and time management skills
