HarmonyCares is seeking a skilled and compassionate Primary Care Nurse Practitioner to join our team of healthcare professionals. As a Primary Care Nurse Practitioner, you will provide high-quality, patient-centered care to individuals in their homes, focusing on geriatric and primary care needs.
Responsibilities:
- Provide primary care to high-acuity patients in their homes, Monday through Friday
- Collaborate with a support assistant to ensure seamless patient care
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments
- Contribute to a positive and supportive team environment, promoting a culture of excellence and patient-centered care
Requirements:
- Current licensure as a Nurse Practitioner in the state where the HarmonyCares Medical Group office is located
- Federal DEA license/eligibility
- Geriatric training/experience preferred
- Active CPR/BLS certification
