The Position:

The Client Resource Coordinator plays a vital role in connecting individuals seeking assistance with the appropriate services and resources to fulfill their needs.

This may involve linking clients to financial aid, disability services, medical programs, rehabilitation services, vocational support, legal aid, housing resources, disaster relief, food assistance, home-delivered meals, in-home support, or other organizations that empower individuals to live independently within their communities.

What We Seek:

  • High School Diploma or G.E.D.
  • A minimum of 3 years of experience in social work or case management, particularly in aging, disabilities, community health, or hospital discharge planning is preferred.
  • Alliance of Information Referral Specialist (AIRS) certification is required; if not certified, you must obtain certification within one year of hire.
  • Strong oral and written communication skills.
  • Experience in customer relations, call management, information and referral, or a related field.
  • Ability to effectively utilize technology to meet job requirements.
  • Valid driver's license and proof of vehicle liability coverage.
