Job Summary

We are seeking a highly skilled Physical Therapist to join our team on a full-time basis. As a Physical Therapist at Concept Rehab, you will be responsible for evaluating and treating patients with mobility, function, strength, and positioning difficulties. You will work closely with families, physicians, and other healthcare professionals to develop and implement effective treatment plans.

Key Responsibilities
  • Evaluate patients for rehabilitation services within 24 to 48 hours of physician's orders.
  • Develop and implement effective treatment plans.
  • Treat patients according to the treatment plan of care.
  • Supervise Licensed Physical Therapy Assistants in the delivery of clinical services.
  • Communicate with physicians, clinical managers, and other healthcare professionals regarding patient progress, problems, and plans.
  • Participate in Plan of Care Meetings and Interdisciplinary Team Meetings.
  • Achieve or exceed minimum productivity standards.
  • Participate in in-service training programs for other staff.
  • Record treatment notes, progress notes, and complete daily billing of services.
  • Instruct patient families and/or facility staff in maintenance programs.
  • Participate in discharge planning.
  • Recommend equipment to facilitate improved delivery of clinical services.
  • Report problems with department equipment.
  • Act as a clinical supervisor for affiliating Physical Therapy and Physical Therapy Assistant programs.
Requirements
  • B.S., M.S., or D.P.T. in Physical Therapy.
  • Licensed in the state(s) where services are being provided as a Physical Therapist.
