Job Summary

We are seeking a skilled Home Health Registered Nurse (RN) to join our team on a full-time or PRN basis. As a Home Health RN, you will be responsible for providing comprehensive nursing care to patients in their homes, collaborating with the interdisciplinary team to develop and implement individualized care plans, and evaluating patient progress and outcomes.

Responsibilities

  • Provide quality, comprehensive nursing care to patients in their homes, as prescribed by the physician in the plan of care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Evaluate patient progress and outcomes, and make adjustments to care plans as needed.
  • Manage nursing treatments and activities to meet the needs of each individual patient.
  • Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.

Requirements

  • Current Texas Nursing License
  • Geriatric Home Health experience
  • Strong organizational and self-management skills
  • Strong and compassionate customer service skills
  • Valid driver's license
