Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Advantmed provider network. In this role, you will be responsible for conducting in-home wellness risk adjustment assessments for Medicare and other populations.

Key Responsibilities
  • Perform annual wellness visits and health assessments on a population with chronic conditions.
  • Deliver patient health education opportunities.
  • Assist in closing quality care gaps (i.e., screenings and labs).
  • Collaborate with our team to enhance care delivery.
Requirements
  • Must have a valid unencumbered NP License for the state you will be working in.
  • Previous in-home Risk Assessment experience is preferred.
  • 3 years of patient care experience is preferred (primary care/adult/geriatric, EMR).
  • Bilingual is a plus.
