Job Title: Permanency Specialist

Department: Foster Care

Position Summary:

As a Permanency Specialist at Gateway Longview, you will be responsible for managing the referral and intake process, developing and implementing procedures, and analyzing outcomes and program trends. You will work closely with program directors and other staff to maintain awareness of current and emerging placement resources and client needs.


Responsibilities:
  • Maintain ongoing contact with referral sources to ensure effective communication and constant flow of referrals.
  • Work collaboratively with program directors and other staff to maintain awareness of current and emerging placement resources and client needs.
  • Participate in Service Plan Reviews and Treatment Team meetings as assigned.
  • Secure necessary intake information and documentation required to complete the intake section of the case record.
  • Establish and maintain regular contact and therapeutic intervention with all parties (children, birth families, foster/adoptive families) ensuring diligent efforts toward the identified service plan.
  • Provide ongoing collaboration with referral sources and community providers.
  • Participate in concurrent planning.
  • Accurate and timely completion and finding of all necessary documentation.

Requirements:
  • Bachelor's Degree with relevant experience in social work or other human services field.
  • Master's Degree in clinically based area preferred.
  • Valid New York State Drivers License and adequate auto insurance.
