As a Nurse Practitioner or Physician Assistant, you'll play a vital role in our Senior Community Care team, providing comprehensive health assessments and collaborating with case management teams to support better care coordination and health outcomes.
Key Responsibilities:
- Work closely with primary care physicians to provide high-quality patient care
- Collaborate with nursing staff and patients' families to ensure seamless care transitions
- Contribute to documentation, coding, and gap closure efforts to enhance patient care
Requirements:
- Certified Nurse Practitioner or Physician Assistant with a national board certification
- Active and unrestricted license to practice in the State of Connecticut
- Ability to lift a 30-pound bag and navigate various dwelling conditions
- Collaborative practice agreement, if applicable in your state
Preferred Qualifications:
- 1+ years of post-graduate experience in Long Term Care
- Understanding of Geriatrics and Chronic Illness
- Proficient computer skills, including electronic medical records
- Demonstrated ability to develop and maintain positive customer relationships
