Responsibilities- Conduct thorough nursing assessments, medication reconciliation, and safety evaluations to develop and implement personalized care plans.
- Administer prescribed medications, obtain blood samples, and provide therapeutic interventions as needed.
- Educate patients and their families on disease management, medication adherence, and self-care techniques.
- Monitor and report changes in patient condition, revising care plans as necessary.
- Document all assessments, interventions, and care coordination details in a timely and accurate manner.
- Participate in regular in-service training and special case orientations to stay up-to-date on best practices.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
- Associate of Science in Nursing degree
- Current RN license in the state(s) assigned to work
- At least one year of adult acute inpatient medical or medical/surgical care experience within the last five years
- Current CPR certification
- Valid driver's license and reliable transportation
- Bachelor of Science in Nursing degree
- Experience providing RN services in home, community, or clinic settings
- Adult medical critical care experience
