Southcoast Health System is seeking a skilled RN-Case Coordinator to join our team. As a key member of our Case Management department, you will play a vital role in coordinating patient care and services along the continuum of care.
Responsibilities:- Collaborate with physicians and multidisciplinary team members to improve patient care through effective utilization and monitoring of healthcare resources.
- Facilitate patient/family/representative achievement of desired outcomes.
- Support utilization functions, including physician-to-physician consultation for correct level of care determination.
- Contribute to denial prevention and compliance with COP requirements.
- Bachelor's Degree in Nursing or other healthcare field.
- Current Massachusetts RN license.
- Certification in Case Management preferred.
- Over five years of related experience, with one year of case management experience preferred.
