Position Overview:

This is a Per Diem role within our In-House Rehabilitation Team, located in East Longmeadow, MA.

Key Responsibilities:

  • Plan, organize, and direct Physical Therapy services in compliance with federal and state regulations.
  • Evaluate patient conditions and create tailored rehabilitation and treatment plans.
  • Focus on enhancing, maintaining, and restoring maximum movement and functional abilities to improve quality of life.
  • Maintain accurate documentation and departmental records.
  • Communicate effectively regarding Physical Therapy programs.
  • Conduct regular screenings to assess residents' needs for intervention.
  • Interpret evaluation findings and communicate them to residents, families, and team members.

Qualifications:

Candidates must possess an active state license or be license-eligible, meeting the educational requirements specified for the license type. A Bachelor of Science, Master of Science, or Doctorate in Physical Therapy from an accredited institution is required.
