Job Overview:

We are seeking a skilled and compassionate Advanced Practice Clinician to join our team in Pottawattamie County, Iowa. As a key member of our interdisciplinary care team, you will provide high-quality patient care, navigate the healthcare system, and connect patients to essential support services.

This per diem position offers a unique opportunity to work with our Optum At Home product, which focuses on holistic care that addresses patients' physical, mental, and social needs. You will be responsible for providing general and preventative care, interventional care, point-of-care testing, patient/caregiver education, and medication prescribing during in-home, telephonic, and virtual visits.

Key Responsibilities:

  • Establish and Maintain Relationships: Foster effective relationships with patients, caregivers, and/or their legal representatives, IDT members, PCPs, other collaborating physicians/providers, and facility staff as needed.
  • Comprehensive Assessments: Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care, including visits to address changes in condition.
  • Patient Management Care Plan: Develop and update patient management care plans as needed, considering hospitalizations and changes in condition.
  • Coordination and Consultation: Coordinate with Landmark Physician Leaders and PCPs as available, ensuring seamless care delivery.
  • Clinical Skills: Demonstrate comfort with basic procedures, such as IV placement, blood draws, injections, foley catheters, nasal packing, and basic wound care.
  • Post-Discharge and Urgent Visits: Conduct post-discharge and urgent visits to ensure patient stability aligns with their goals of care.
  • HEDIS Measures and Quality Initiatives: Participate in HEDIS measure campaigns and quality initiatives to promote high standards of care and improve care management and delivery.
  • Territorial Coverage: Cover assigned geographic territory, potentially requiring substantial drive time in rural areas.

Salary: $150,000 - $180,000 per year, depending on experience

Requirements:

  • Nurse Practitioner (NP) or Physician Assistant (PA) Program Graduation: Graduate from an accredited program
  • APRN or PA Licensure: Hold current APRN or PA licensure with unrestricted license in good standing
  • Board Certification: Be board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC)
  • Prescriptive Authority: Possess active prescriptive authority in the state of professional licensure
  • Computer Skills: Demonstrate solid computer skills, including electronic medical records
