About the Role

We are seeking a skilled Nurse Practitioner to join our team at Ennoble Care, providing primary care to homebound and palliative patients in Loudoun County, Virginia.

Key Responsibilities:

  • Deliver high-quality care to patients with chronic diseases, working under the supervision of a physician.
  • Conduct thorough assessments, including physical examinations and medical histories, to develop effective care plans.
  • Communicate effectively with office staff and clinical operations across various platforms.
  • Provide patient-centered care to a panel of patients, fostering trust and consulting with patients, families, and caregivers during visits.
  • Order diagnostic testing as medically necessary and prescribe medications, supplies, and interventions.

Requirements:

  • Board Certified, with a valid NP or PA certification in the state of Virginia.
  • Strong clinical skills, with the ability to accurately and confidently treat, assess, and diagnose.
  • Positive demeanor and strong people skills, with the ability to problem-solve and operate independently.
  • 1-2 years of primary care experience with geriatric patients, with hospice experience highly preferred but not required.

Compensation and Benefits:
