Key Responsibilities

The Hospice Liaison, functioning as a Registered Nurse (RN), will serve as the crucial link between the healthcare facility and the hospice team. This role involves conducting inpatient assessments for hospice patients, providing expert consultation for pain and symptom management, and delivering educational sessions to healthcare providers. The RN will also perform pre-admission evaluations to determine the suitability for hospice services and initiate the admission process by informing and educating patients and their families about hospice care and its offerings. The liaison is dedicated to ensuring high-quality patient care and exceptional customer service. Additionally, this role assists the Director in formulating policies and procedures aimed at enhancing patient outcomes.

Qualifications
  • BSN or MSN from an accredited nursing institution is required.
  • Prior clinical nursing experience is preferred.
  • Active RN license in Maryland or a compact state, along with Basic Life Support certification for healthcare providers, is required.
Education
  • Bachelor's degree from an accredited nursing program is mandatory.
Experience
  • Experience in hospice care is essential.
Licenses and Certifications
  • Registered Nurse licensure in the state or compact state is required.
  • CPR certification (including BLS and NRP) from the American Heart Association or American Red Cross within 90 days is required.
  • Additional unit or specialty certifications may vary by department.
Conducts thorough admission assessments and engages with patients and families to build rapport and trust. Develops a comprehensive hospice care plan that includes appropriate physician orders for effective pain and symptom management.Clearly communicates the limited stay for symptom management and collaborates with Social Work to create a discharge plan once symptoms are stabilized.If a patient requires admission or transfer to home or another facility, coordinates with the Hospice Intake/Referral department.Conducts death visits and assists in assessing family needs for bereavement services post-patient death, relaying necessary information to bereavement staff. Completes death documentation in the electronic medical record (EMR).Ensures timely completion of all electronic documentation in both the hospice EMR and hospital EMR.Manages cases for patients receiving General Inpatient (GIP) level of care, ensuring daily Skilled Nursing Visits (SNV) are provided. Documentation of daily SNV includes symptom management, interventions, and discharge planning.Participates in the hospice on-call schedule as needed.
