Our Continuum Hospice team is seeking a compassionate and skilled Registered Nurse to provide exceptional end-of-life care to patients and families in the Fall River area.

We are committed to delivering the best care possible, and our RN Case Manager will play a vital role in achieving this goal. As a member of our interdisciplinary team, you will work closely with patients, caregivers, and other healthcare professionals to develop personalized care plans that meet the unique needs of each individual.

Responsibilities:

  • Empower patients and families through education and support, ensuring their comfort and quality of life remain paramount.
  • Collaborate with our Clinical Director to assess patient eligibility and suitability for our specialized hospice services.
  • Deliver top-notch care rooted in proven principles and techniques, easing pain and bringing solace to those under our care.
  • Empower patients and families through education, encouraging their active participation in creating personalized care plans that align with their goals.
  • Join our team in sharing the responsibility of call duties, providing support and comfort to patients and families in their time of need.

Qualifications:

  • Hold a current, unencumbered license as a Registered Nurse.
  • Minimum one year of experience as an RN in a medical surgical/acute care setting, with hospice experience a plus.
  • Be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintain current CPR certification if required by state.

Compensation:

$48.00 per hour (average pay range), with pay determined based on experience.
