Post-Acute Transition Specialist

We are seeking a highly skilled Post-Acute Transition Specialist to facilitate the safe and timely transition of clients from acute care to alternative levels of care.
About This RoleThis role involves working closely with healthcare teams to implement and manage discharge plans, ensuring seamless transitions for patients. As a Post-Acute Transition Specialist, you will be responsible for:
- Processing and facilitating timely discharges/ transfers of clients from hospital care to identified post-acute settings.
- Notifying care coordination team members if patients or caregivers demonstrate any inability or concern to manage their post-acute plan or responsibilities.
- Facilitating and implementing care plans with proposed interventions in collaboration with healthcare teams.
- Participating in performance improvement projects and performing data collection for measurement of projects as assigned.
As a Post-Acute Transition Specialist at Banner Health, your key responsibilities will include:
- Documenting all interventions in patient medical records both timely and accurately, including all elements of the discharge plan.
- Performing transfer of accurate, pertinent patient information between all appropriate entities of the post-acute care continuum.
- Assisting and supporting patients and families in making appropriate arrangements for post-acute plans.
- Serving as an intermediary when providing community resources to patients, caregivers, and families.
To be successful in this role, you will need:
- A Bachelor's degree in social work or related field.
- Knowledge of government/community agencies and resources, such as Medicare/Medicaid, long-term care, or other applicable resources/services.
- Effective communication and customer service skills, human relation skills, and time management skills.
- Ability to work flexible hours and weekends on rotation.
- BLS certification (not required for employees working in the Insurance Division).
Previous experience in a healthcare service setting, interacting with patients and families, usually obtained through work in social services, as a licensed practical nurse, or in a discharge planning setting is preferred.
Compensation and BenefitsThe estimated salary for this role is around $65,000 - $80,000 per year, depending on location and experience.
