Job Summary

ConcertoCare is seeking a skilled Advanced Practice Provider to deliver high-quality, patient-centered care to seniors and older adults with complex care needs. As a key member of our multidisciplinary care team, you will provide compassionate, individualized care in the comfort of patients' homes, leveraging our proprietary technological systems to support seamless care coordination.

Responsibilities

Provide comprehensive care, including urgent care, chronic condition management, and transitions of care support
Collaborate with the care team to deliver social determinants of health interventions, behavioral health evaluation and management, and complex medication management
Deliver exceptional clinical care in-person and virtually, with a focus on patient engagement and experience initiatives
Maintain accurate and timely clinical documentation, including medication reconciliation and patient care-related activities
Participate in care team huddles, operational huddles, and clinical partnership meetings to support care coordination and goals of care

Requirements

Nurse Practitioner or Physician Assistant with graduation from an accredited health professions school and successful completion of national board certification
Active licensure in applicable state(s) and DEA licensure, as applicable
Advanced training in geriatrics and/or palliative medicine preferred
Certification in basic life support (BLS) required
Clinical experience caring for aging adults and adults with complex care needs required
Ability to travel as needed to locations with limited access to public transportation
