Part-Time LVN

We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team on a part-time basis. As an LVN, you will provide high-quality patient care and support to individuals in need of assistance with daily living activities.
Responsibilities- Provide one-on-one care to clients, following their individualized care plans and maintaining their dignity and independence.
- Monitor clients' vital signs and report any abnormal findings to the case manager and physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, and document the results of these interventions.
- Complete routines established by other care professionals, such as physical therapists, occupational therapists, and speech therapists.
- Ensure infection control policies are maintained at all times, report any possible communicable diseases or infections, and implement infection control procedures.
- Accurately document nursing actions, communicate with family members, case managers, and physicians, and document progress and outcomes for established goals.
- Order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
- Ensure all patient needs are met in accordance with physician instructions.
- Monitor incidents involving individuals, complete incident reports, and inform case managers of any incidents.
- Ensure all medical equipment is maintained and report any faulty equipment to the Durable Medical Equipment department for repair.
- Current license as an LVN in good standing from the state(s) of practice.
- Current Cardiopulmonary Resuscitation (CPR) certification.
- Ability to work in a home-like setting.
- Ability to bend, stoop, push, pull, sit, and walk for periods of time.
- Able to utilize proper body mechanics while lifting 50 pounds.
- Knowledge of individual service plans, including understanding of client rights and health needs.
- Must have completed post-secondary coursework to facilitate licensure as an LVN.
