LPN - Licensed Practical Nurse

We are seeking a skilled LPN to join our team at Adoration Home Health and Hospice. As a Licensed Practical Nurse, you will provide high-quality care to our patients in the comfort of their own homes.
Responsibilities- Provide 1:1 care to clients, following their individualized plan of care and maintaining their dignity and independence.
- Monitor clients' vital statistics and report any abnormal findings to the Case Manager and Physician.
- Administer medications, feedings, oxygen, and ostomy care as needed, and document the results of these interventions.
- Complete routines established by other care professionals, such as PT, OT, and Speech Therapists.
- Ensure infection control policies are maintained at all times, and report any possible communicable diseases or infections.
- Accurately document nursing actions, communication with family, case manager, and physicians, and document progress and outcomes for established goals.
- Coordinate or order supplies, medications, and feedings as needed to ensure adequate amounts are on hand.
- Ensure all patient needs are being met in accordance with the instructions of the physician.
- Monitor incidents involving individuals, and ensure incident reports are completed timely and accurately.
- Ensure all medical equipment is maintained, and report any faulty equipment to DME for repair.
- LPN/LPT/LVN degree from an accredited college of nursing, with current unrestricted registration and license in the state of practice.
- Valid driver's license, acceptable driving record, and proof of car insurance.
- CPR professional certification.
- Experience in administering medications.
- 1+ years of nursing experience preferred.
- Home health care experience preferred.
- Able to lift and carry items weighing up to 20 pounds.
- Able to bend, stoop, push, pull, sit, and walk for periods of time.
- Must be able to communicate both verbally and in writing.
